Getting Started With Senior Care At Home

When you’re ready to get started with home care services, we’ll schedule a free in-home consultation with one of our RN Care Managers. This initial meeting helps us assess your loved one’s physical and mental status, gather health information, and learn about their daily routine and preferences. We want to get to know your loved one and develop a comprehensive care plan to meet their needs. Every person is different. Our customized care plans help ensure that our in-home care service is personalized and tailored to meet your specific needs.

Some of the topics discussed at the consultation include:

  • Personal goals and preferences such as nutrition, exercise, getting out of the house, or engaging in hobbies and social opportunities
  • Mobility and safety concerns
  • Personal care needs such as assistance with showering, dressing, and toileting

  • Chronic and acute medical conditions
  • Cognitive diagnosis or concerns

We consider skills and experience required as well as personality characteristics and personal interests and preferences. We are very intentional about this process because we have a vested interest in a successful relationship.

Our caregivers are professionals. We are selective in our hiring, and we invest in ongoing training and supervision to help ensure our team delivers exceptional care.

Starting Service

Once we establish a start of care date and confirm your caregiver, your care manager will set up a time to introduce your caregiver. We review your care plan and schedule with your caregiver prior to bringing them to your home as part of their orientation. We will check in with you as well as your caregiver after a few days to hear your feedback and address any concerns. After a few days getting acclimated, usually our clients tell us things are better than they expected. Our goal is to make your life easier while we promote independence and quality of life.

Through the duration of your service with us, we will be by your side. Your care team including caregivers, care manager, and staffing coordinator are available to support you, answer questions, adjust your schedule, and update your care plan as your needs may evolve.
